After scheduling your initial visit, we will mail you information about the practice as well as your procedure(s) of interest. Please fill out the enclosed forms and bring them along with your insurance card to your appointment.
We hope to make your visit as comfortable for you as possible, so please let us know of any concerns you may have before your scheduled appointment.
To efficiently accommodate all of our patients' needs, we request at least 24-hours notice in the event you need to cancel or reschedule an appointment. This gives us adequate time to reschedule patients appropriately and avoid disruption in patient care.
Not notifying us in advance will result in a $25 fee. No-shows to appointments will also be charged a $25 fee.
Your appointment time is reserved especially for you. Adequate time is necessary to give all our patients the personal experience that we require to maintain our level of care. We request that any cancellations or changes are made at least 24 hours in advance.
In the event we do not receive 24 hours notice, the patient will be billed 50% of the service fees to compensate for the loss of time to our service providers.
As a courtesy to following guests, late arrivals may be asked to reschedule, or may have the option to receive only the remaining available appointment time and will be charged in full.
Please notify our front desk staff so they can help you arrange the best appointment time.

Financing is available through CareCredit®, as well as several local financial institutions. Please ask your patient care coordinator for details.
